Housing Support Worker

Eden Health Care Services in partnership with the Winnipeg Regional Health Authority provides community programs as well as services and supports specific to individuals residing at Concordia Village IV. Concordia Village IV is an affordable apartment consisting of 45 units of 1, 2 and 3 bedroom suites - located at 1055 Molson Street, Winnipeg.

The housing support worker will provide support services to individuals living with mental health concerns in maintaining their housing and in their work towards recovery. The housing support worker provides, rehabilitation and support functions including skill building, and linking to and maintaining formal and informal support networks. This is a full time position with flexible hours and may require some evening and weekend hours.

Responsibilities include:

• Collaborating on recovery plans with client team
• Taking initiative to interact with client regularly
• Planning activities to support recovery plan for client
• Accompanying client to appointments when appropriate
• Connecting clients with supports
• Writing case notes.

Qualifications:

• Commitment to the vision, mission and values of Eden Health Care Services
• University or College education in Human Services or high school diploma and related experience
• Knowledge of mental illnesses/mental health issues
• Good communication skills, both verbal and written
• Ability to work independently as well as part of a team
• Good problem solving and decision making skills
• Knowledge of community resources
• Excellent interpersonal skills
• Subject to Criminal Record Check, Child Abuse Registry and Adult Abuse Registry checks.
